We use a one page form that offers three options...accept new (rate, term); propose change of stated terms, will require approval of PM prior to acceptance; and decline new terms, please consider this our X days notice of vacating per State laws.Add a reminder that Security Deposit needs to be increased to match the new rate; and provide a deadline for returning the completed and signed addendum (this should be 2 - 3 weeks prior to end of current term, depending on local law).As long as you reference the name of the original Rental Agreement and it's original date, by keeping the original in file with your other ancillary docs such as LBP, they all remain in effect.
