You obviously don't pull any permits.  :-\- It's really too difficult to say without seeing all the windows.

Has anyone had a permit issued lately in Houston when rehabbing an old house?

This is pretty straightforward, but if someone was to build residential units above the ground floor retail unit (permitted; and no ground floor residential units are permitted in the B3 designation), would the retail unit on the ground floor count as a unit when considering the minimum lot area per unit rule?

Sometimes, it can be as easy as doing a minor-subdivision of land and changing zoning or just obtating exceptions and permits.
