How did the accident happen was someone on a cell phone, reading a newspaper, etc.?

My thinking is it's like the cell phone, the technology just keeps getting smarter and smarter, so eventually will all those that have adjusted to technology want to carry it on to their kitchens?  

As to cell phones, keep the batteries charged!
