I think this is deserving of class action suit as the assessments are so far off in Baltimore city that it is criminal.

He got off the criminal charges, but was found guilty in civil court to the tune of over 5M.

I would use the one form as long as you can get the information you need from both of them to adequately check their, finances, credit and criminal background. 

Pets, smoking, early termination, property maintenance, criminal actions, etc should all be considered.

its not a great house at a great price if noone is buying. and I will say that I have been part of neighborhoods changing for the better. however it's not something that can be easily done it would require moving into the neighborhood and an effort spanning years. and you can't do it alone you need other neighbors on board. it's not easy to gentrify the neighborhood it's not something you do it's something that happens. at least if you're close to the sheriff station it will discourage criminals and people with warrants outwhereare  the younger families buying homes and price your home under that.

This is a good example of why going forward you make sure all unrelated adults over 18 are qualified individually (I state unless a dependent on another applicant's tax return, but background check still required), and a criminal/background check should have told you he has a current record so would not have been a good candidate for a tenant to begin with.    

Unless you did something dumb, like label the box "Apt 206", no criminal is going to steal the keys and try every door in the complex.

In Ohio, we have 2 courts in each local that handle most of the criminal and civil activity.  

They also won't be sent to jail as this is not criminal.

The state is like, here is a list of people most likely to be motivated to sell and would welcome your help... but if you use it you are a criminal!
