I've had central A/C units torched through bars/cages and stolen to all my new doors and hardware pulled off every doorway taken in minutes in these areas.

This included things like installing transition pieces in the doorway between laminate and wood flooring "because the change in surface texture was a tripping hazard", replacing a concrete slab in the drive (Which the tenants do not use) because there were cracks, scraping and repainting minor cracked/peeling paint in the halls "because it was a health hazard", installing GFI outlets which the other landlord had never been cited for, and a number of other things that clearly had not just developed since the previous inspection.

You also need full control of the property so you can make improvements (e.g. handicap doorways or bathrooms).Any house design could theoretically work, but a single-level home is ideal for handicap accessibility.

You don't usually plumb this into a drain; you just use a few feet of pipe or hose to get the drip away from the doorway or whatever.

Though not purchasing quite yet, we are all evaluating options, making sure our next homes will be accessible - no stairs, wider doorways, elevators (if we select to purchase in a larger multi-unit building), with shops and healthcare nearby, etc. 

Your contractor can help you add the doorway, as well as make sure other things (electric etc) are up to code.
