@Jason May poor MSA, declining population, declining job growth, rising crime, rising vacancy, poor schools, proximity to class D areas/railroads/waterways, floodzones, high taxes, knob and tube wiring, poor foundations, extreme termite damage, generally bad layouts, raccoon (or other animal) infestations... just to name a few.

As a general rule:VA Loan - The WDO needs to pretty much be perfect.FHA Loan* - Infestation must be treated and all visible damage must be repaired.Conventional Loan* - Nobody cares.
