Also, no go if it was near an onramp or stop sign/light where cars were constantly revving engines to accelerate ... steady traffic flow is much less intrusive.

Water intrusion through no fault of the tenant that causes mold would be on owner most times although tenants failure to report water intrusion (or any maintenance issue like a leaking pipe) that results in mold makes them somewhat culpable for the resulting additional damages caused by their neglect (mold).

A few sources I've read suggested that tree root intrusion is the only problem that a scoping will catch because cast iron and clay pipes either rust or crack from the outside in, so unless the line is actually broken at time of inspection an impending break won't be seen.
