I have fortunately had very little interaction with insurance companies and claims so don't know the details of when they quit writing, although I have heard that making a claim is a strike against you regardless of whether they find coverage or make a payment.

Think about a rental strike that goes big where the court gets tied up with landlords trying to evict but there are so many it can take forever, meanwhile you have no income.

That strikes me as the biggest to FIRE.
