The odor was horrible (and is still lingering).

I wish I'd know that you should try to get rid of the odors (with any/all methods above) BEFORE  you replace floors, paint etc.

Rip out carpeting & padding, If the odors have been absorbed into the walls, you should probably use Kilz Max. 

If the previous smoker was a heavy indoor smoker (probably the case in light of the lingering smell) then the odor can penetrate sheetrock more than most people realize. 

The odor is VERY strong and has small piles of mud scattered throughout.

Shellac seals in the odors on wood.
