It's also important to add that one must provide a legal size, stamped envelope (two if serving two tenants, even if they reside in the same household) plus all fees in cash and the appropriate number of copies for filing (the original and two for one person, the original and four for two....keep copies at home as repeatedly filling this out causes hand cramps!
