I usually gut bathroom to studs and have plumber replace any galvanized or cast iron plumbing with pex and pvc, make sure exterior wall has insulation and plastic, check my wiring and move if necessary, install new fart fan, new drywall, tub, vanity, medicine cabinet, towel bars, usually run tile from top of tub to ceiling, new toilet, vinyl plank flooring.

From my limited belief, the law technically covers double closes this but as @Irfan Raza stated there has been no REAL enforcement, if anything it is just causing doubt in newcomers in the market which ironically was the starting motivation of this thread.
