I'm curious to hear how these are set up in newer developments where city water and sewer are too far away.

I wish I could help on this, but now you know one thing to look out for (rural properties are sometimes VERY tough for lenders to get behind).Advice would be here to force their hand in communicating, and if they're not living up to your standards, proceed to find another lender that will.

with maybe the exception of a parcel that is clearly served by city Sewer Water and have power and Tele / internet to the lot lines.. and you have double checked the hook up or Systems development charges and if they have capacity.. keep in mind many parcels in subdivisions that were basically built as recreational subdivisions decades ago.. ( think GLEN GARY GLEN ROSS and for sure watch that Movie is exactly how the industry was in those days) the original infrastructure was allowed to be built to maybe serve 20% of the development that was legal and now you have moratoriums and assessments etc etc..

I'm in Nothern NJ and 1.5x is standard however on higher priced rentals with a broker fee, I will take 1 months rent for SD (or the broker takes a smaller fee).

The due on sale clause will be standard for every lender that I know of. 

Seems like a pretty standard situation there. 

@David Neubauer The standard is now shifting toward this 5% down program.

New flooring, new cabinets, new tankless water heaters in 3 of the 4 units.

Does it have a drinking water well that has clean drinkable water for human beings?

I think a lot of this has come out due to the low standard of brokers out there that give the industry a bad name unfortunately... its so easy to get a license nowadays.
