I didn't run numbers, but my "feeling" is that your 1 and 2 are sort of contradictory / uneven application of concept. 

Ideally, you'd take a full loan application, run the loan through automated underwriting and the automated underwriting findings will tell you exactly what you need to obtain from your client as far as financials go.   

BTW, you just said "next" to a nice young engineer who just got a new higher paying job and is an A+ type applicant.

They should explain why they took a permit application from someone not authorized to submit one for that company.

I'd stated the occupancy limit on the property is 4 adults and was a bit unprepared when he filled out the application with 6 total people (himself, wife, and 4 kids).
