IRS/State can assess you interest/penalties for failing to report it on your tax return if they find out.You also face losing the depreciation when you sell the property.You may also be benefiting by filing an amended return.When filing the amended return - you have to factor in that the mortgage interest and real estate taxes that you originally deducted on schedule A would now go on Schedule E.Another reason for amending is if you plan to report the rental income in the future - there is a slight chance that it will be a red flag if you report that you purchased it in 2013 but just started reporting rental income.You will not be required to register with the AZ secretary of state if you did not own the property through an LLC.

Check the NY Secretary of State website....you can usually search entities to get names of members, mailing addresses.
