Generally, if there is a shortage, you can apply for the comprehensive permit.  

As far as income requirements, we use total household income which is pretty common.

We use Proper, it is more expensive, but it is a lot more comprehensive from what I can see.We are not in the PF area and have not had to use the insurance but from others that have, Proper is speedy on claims.
