Assisted living, memory center, continuing care, rest home, independent living, ambulatory care are all terms for what you might be looking for. 

It is a hub network for the University of Pittsburgh Medical Center (UPMC) and Allegheny Health Network.

If you can handle your own paperwork without needing a compliance component, front desk appointment center, etc. then that may be an even better option.
