Where do you use your LLC name: in Line 1 or Line 2? do you write the name of holding/parent LLC or subsidiary/operating LLC.

For EIN do you use EIN of holding/parent LLC or subsidiary/operating LLC?

I have formed a holding/parent LLC company (with me and my spouse as 50/50 members) and a subsidiary/operating LLC company which is 100% owned by the holding/parent LLC

I am filling out a W9 form for a property management company for a property I am closing under my 

subsidiary/operating LLC

On Line 1: Do I write My Name?

On Line 2: Business Name/disregarded entity name: Write the name of subsidiary/operating LLC

On Line 3: I have checked Individual/sole proprietor or single-member LLC. Is that correct?

On Part 1: For TIN, do I enter EIN of subsidiary/operating LLC or my SSN?

I thought you should have your basic team and finances ready before you contact an agent. so that if they get you a deal that you will be able to act fast.
