How to describe this repair?

When an addition was added to this home, the foundation was (what I describe as) chipped away.  The addition is about 4 inches lower than the rest of the house.  Carpet and padding were simply placed over this.

I had the floor leveled in the living room. The only other spot left is a door opening to a hallway and vinyl tile "rolls" over it.  How do I put the repair to the living room in a very short sentence so the appraiser will understand what the heck I mean?
