Has anyone used rentapp for rental application or the best way to go is traditional paper forms.

You can actually write your own.  Then use some service to get the BG, Cr, and eviction reports.

CR reports require:  full name, DOB, SSN, last known address, and previous address.

W/o the first three, you get no reports at all.

ask residence questions:

• current addr, previous addr
• name and phone num of each
• WHY are you moving

ask family questions: 

• names living with you, pets
• number / license for cars
• phone / email addresses

ask employment questions:

• where & how long
• manager name & number
• current income for each adult

at the bottom, be sure to state

"I declare the above statements are true and correct and agree that they are part of the rental / lease agreement.

I hereby authorize a background check, verification of references, employment and a credit check as a condition of occupancy;
