Does anyone have a template of a quit notice for habitual late payment of rent?

You shouldn't have a habitual late paying tenant. Do they pay late each month or are they behind by more than one month?

They pay every month but 10-20 days late. I go to court and file for eviction on the 6th of the month. (I have them on a grace period till the 5th of the month).

Here is what I used:

1. Pursuant to a written lease (the "lease") dated xxxxx, you are a tenant for the premises described as xxxxxxx (The "Premises"), of which you now hold possession.

2. You are in violation of the lease by being habitually late in paying rent. Since the start of this lease you have been late xxxxx.

3. Due to the above mentioned reason, the Landlord hereby elects to terminate this Lease.

4. You are given at least 30 days notice to quit the Premises in compliance with the applicable laws for the State of New Jersey. Accordingly, the Tenant must vacate and surrender the Premises to the landlord by xxxx. (If all rent are paid up to date). If rent is not paid eviction process will start prior to that date.
