rental lease - do you seek SSN and listing household members

I have 2 questions.... 

1. I have a lease template for rental property and the rental agreement does not include/ask for tenant's SSN. I did the background check by asking the applicant to provide their information to the 3rd party provider and the 3rd party provider produced a report and everything looked good - no eviction records, good credit history etc, but the 3rd party provider does not share the SSN of the applicant with me (which I understand). However, as part of the rental application do your normally ask for SSN of the tenant and if they choose not to provide/disclose, can you still rent to them?

2. The applicant shared with me that the household is 5 people (1 applicant + 2 more adults + 2 kids). The applicant was background screened and checked and is the only one who has the SSN. 2 other adults don't have SSN and they have traveled to the US on a visa. How and where do you list 2 adults in the rental lease agreement?
