Ownership of LLC filing with state/Operating agreement for Oregon

Hello - I'm setting up an IRA/LLC, and I have my money transferred to a custodian (IRA Services Trust Company), I have setup a few llc's before so I'm doing this part myself. Is the trust company listed as owner and myself as manager? Does the IRA ownership need to include acct number, or just "IRA Trust Serves Company"? Any other pitfalls to avoid? I would like to purchase a professional operating agreement for the state of Oregon, so let me know if anyone has that. Thank you!
