Good Saturday morning BP Forum!

I am in a position to begin building up my investor database beyond a couple of tier 1 relationships. I have had investors from other parts of the country reach out to me in the last couple months and while I took their contact information I didn't really have a good set of questions to ask them to get to understand them. This is mostly my fault for not exploring these options earlier and learning as much as possible. I am trying to correct this issue now and one thing that I need is an investor evaluation form. Something that lets me know their deal type interest, risk level, etc. I know @Matt Faircloth in his book Raising Private Capital mentions using a form for this and I wanted to take that approach. My issue is that being new, I feel if I create this from scratch I am going to miss way to many items. 

Does anyone have a good sample of one of these forms that they are willing to share? Also any tips on things to make sure are included would be appreciated. Thank you for any feedback!
