Who’s fault is it.. realtor or lender?

My brother was preapproved for a mortgage, found a condo, made an offer. Offer accepted. Condo went through appraisal process. Now he just found out, the condo is not FHA approved. Everyone trying to blame each-other. Who's fault is it really?
