Cancel California Purchase Agreement

Hi all,

I need some help in cancelling my California Purchase Agreement. I am in the inspection period and have a valid excuse to cancel the transaction according to the buyer's inspection advisory.  No contingencies have been lifted so I have the right to cancel. Unfortunately, my realtor is not sending out the cancellation form and I want to be proactive about this. Can I send this form out myself and if so does the seller need to sign off on the form?  If the seller refuses to sign off on the form then what happens?
