Ontario, Canada Rental Application

Hello, 

This question goes out to people that rent out property in Ontario, Canada.

Is there a standard rental application that we must use when screening our tenants in Ontario? I believe that we have to use the lease provided by the Landlord Tenant Board, but I can't seem to find info about a standard application. I just want to be sure.
