Landlord's 30 Day Notice- Filling out, Norarizing

Here in Chicago, Illinois I need to serve my tenant a 30-day notice.  It is important that it be completed correctly, otherwise its validity could be questioned by an attorney or judge. I plan on delivering the notice to the tenant in person.   At the bottom  of the form there is an "Affidavit of Service"where I swear that I have served the ____tenant on ____ date.   This Affidavit is notarized by a Notary Public.  Does this portion get signed and notarized before or after I serve the tenant?

Are there any other tips for correctly completing this form.

Mike it needs to be notarized prior to serving tenant.

Of course you know you need to sign in front of notary, or they will not notarize.

Good luck, by the way is the tenant late with rent ?

A bad tenant ?
