Sample turn-down letter

I have several applicants that didn't make the cut so to speak. Everything from poor background checks, to not enough, and to high of a DTI. I want to be as generic as possible but still professional. I am aware of all the laws regarding how careful you need to be when declining an applicant.

Does anyone have any specific templates they use in these situations?
