Rental Increase Letter Templates

I'm increasing the rent on a rental my tenant would be staying in for one year in April 26 2015. I'm interested in hand delivering the letter having him sign my copy (indicates it was received 30 days in advance.) The length of time affords both of us options: If he agrees I would like new lease ready and signed by Feb. 26, 2015 or send him another letter letting him know lease will not be renewed and date for vacancy/ eviction. Do anyone have a template I can glean some ideas from.
