Non Military Affidavit for an eviction in Florida?

I'm evicting a tenant in Florida, and was just told by the court today that a non-military affidavit is required when a tenant hasn't responded to the 5 Day Summons. To proceed with the Default of Judgment and Writ of Removal, I need to submit this notarized document. Basically, the burden of proof is on the landlord that the tenant hasn't responded because they may be deployed.

The court clerk said that particular document isn't in the court's online eviction packet yet, but judges are delaying default judgments now if it's not included. I found the form online.

Has anyone else come across this?

    Yeah, this is the link and steps:

    1. Go to this link (warning, the site is slow many times, so don't expect it to load quickly):
    https://dmdc.osd.mil/appj/scra/welcome.xhtml

    2. Choose the "Single Record Request" and click "Submit".
    If you do the multiple request, it'll ask you to upload a file, which I'm unsure on....so just look up people individually.

    3. Fill out the form. The entries with asterisks beside them are required, but I go ahead and add their first name too. Click "Look Up".

    4. It will create a PDF file that you can either just open or you can save to your computer.

    You definitely have to have it notorized?
