How to convert a deed to my name

Inherited a home from my deceased parent. Probate completed have decree and have document to record transfer with town. My question is two fold.

1) There are two deeds for the one property consisting of adjoining land. Do I need to make into a single deed and how do I do that?

2) I assume, even after the transfer filing, these deeds will still reflect my parents names. How do I convert them to my name?

Basically what I asking is, do I need a lawyer for all this is or can I do it myself and how to go about it.

The court Order will designate who property is to be transferred to (you). 

Typically, you would have one deed naming the personal rep/exec/admin of the estate John Smith, pursuant to Order, court name, case No. 123456 grants to Digger Odell (or your legal name, if different). 

The deed would reference parcel #1 by legal description and assessor tax ID and parcel #2 same way.

Suggest you pay a title or escrow company to help you. Or, a legal scrivener such as "We the People" which is a franchise operation.
