Private money/lender - Documents, forms, declarations etc...

Hi BP,

A family member has finally accepted to be my private lender and loan money to my LLC.

What do I need? 

Is there a specific document do we have to sign? Does it have to be approuved by an attorney? Any declaration of the interests to the tax department? Well, what are the next steps ? ...
