I am working on a cash-out refi for a duplex in FL. Best I have found so far is 75% LTV, but they require business entity documentation. Anyone have experience with lenders that offer 75%+ LTV on a cash out refi on a duplex that do not require business entity documentation?

75 LTV that requires a business entity sounds like a commercial lender. Is this from a typical bank? Keybank, Chase, Bank of America? A bank or a mortgage broker ought to be able to find a 80% LTV in your name.

If you want to explore the hard money space, both Civic Financial and Kiavi (formerly Lending Home) offer products up to 80% LTV. As to the business entity documentation, I believe that has more to do with who owns the property on title (whether you own the property in an entity or personally). Each lender will have their own guidelines, but most have a rhyme or reason as to why they ask for certain documentation.
