Oklahoma quit claim deed language

I need to fill out a quit claim deed in Oklahoma and I found the paperwork on county site but I do not know what goes where it says parts with a blank after them. Thanks for looking

 then you really need to pay a title company to do all this if you do not know how to fill this out.

Most of the local title companies will do this deed for you and charge about $150.

Thanks for reply’s, contacted several title companies none of them offered that service , I  am having a lawyer do it for me.
